The Bridge to Care Consultation Team provides palliative care to individuals coping with end of life issues. Addresses physical, emotional and spiritual needs in order to relieve suffering and improve quality of life.

 

Services can be provided at home, hospital, residential care facility, or hospice, and can include grief and bereavement counselling, equipment and supplies in the home and access to a hospice residence or an acute palliative care unit. Support is extended to family members.

  • To allow any person suffering from an incurable disease in their final stage to remain at home as long as possible, near his loved ones, and pass away there if they so desire, by providing all the care, services and support required by their state.

  • To allow caregivers who want to keep their loved ones in a terminal phase at home, to get them the help, support, training and information they need to best assist the person at the end of life.

DEMENTIA

As populations age, caring for people with dementia has become more common. Elderly caregiving may consist of formal care and informal care. Formal care involves the services of community and medical partners, while informal care involves the support of family, friends, and local communities, but more often from spouses, adult children and other relatives.

In most mild to medium cases of dementia, the caregiver is a family member, usually a spouse or adult child. Over time more professional care in the form of nursing and other supportive care may be required, whether at home or in a long term care facility..

Community nursing services are provided by a licensed nursing professional to clients in the community who require acute, chronic, palliative or rehabilitative support.

 

Services include assessment and nursing interventions such as education, wound care, medication management, chronic disease management, care management, post-surgical care and palliative care. 

 

Generally, community nursing services will be provided on a short-term basis and community nurses assist clients and their families to be confident in taking over care at home.

 

Community nursing services may be provided in a variety of settings such as clinics, the client’s home, assisted living residences, family care homes, group homes, or other community settings.
